Job summary

The Registered Nurse will provide comprehensive nursing care to elderly PACE program participants, including assessments, treatments, medication management, health education, and coordination with interdisciplinary teams. Responsibilities

The Registered Nurse provides comprehensive nursing care, assessments, and health counseling to elderly participants in their homes or at the PACE center. They also supervise in-home caregiver staff and collaborate with the interdisciplinary team to ensure effective care plan implementation and risk mitigation.

Qualifications
