Responsibilities

The CCM Manager oversees the daily operations and performance of Chronic Care Management and Principal Care Management programs. This role leads a team of clinical coordinators, ensures compliance with CMS guidelines, and drives operational excellence through process improvement and performance management.

Candidates must have a bachelor's degree and at least 3 years of experience in healthcare operations or clinical program leadership. Strong knowledge of CMS documentation and billing practices, along with proven team management skills, is required.
