Responsibilities

The RN Case Manager coordinates and oversees interdisciplinary plans of care for patients in home care and hospice environments. They provide hands-on clinical care while collaborating with physicians, social workers, and caregivers to improve patient health.

Qualifications

Candidates must have an Associate's Degree in Nursing, a valid Wisconsin or compact state RN license, and a valid driver's license. BLS certification is required, while a Bachelor's degree and experience with Epic software are preferred.
