Job summary

We are seeking motivated LPNs with experience in Home Health Skilled Nursing Visits to join our team in Grand Junction. In this role, you will visit clients in their homes, assess their needs, develop care plans, and educate patients. The role requires providing skilled nursing care, maintaining documentation, communicating with healthcare teams, and adhering to regulatory requirements.

Responsibilities

Qualifications
