Job summary

Seeking Pediatric Licensed Practical Nurses for overnight shifts on our Transition Team in New Castle County, Delaware, to deliver complex home care and support the transition from hospital to home care. Responsibilities include providing quality nursing care and training to ongoing care teams, with full benefits, training, and flexible scheduling.

Responsibilities

The LPN will provide complex home care to pediatric and adult clients while facilitating the transition of care from hospital settings. Additionally, they are responsible for educating and training the ongoing care team to ensure high-quality service delivery.

Qualifications

CPR certification home care LPN pediatric care training and education trauma/ventilation experience

Candidates must hold a current LPN license and possess specific experience in NICU, PICU, ICU, or home care trach/vent settings. A current CPR certification and the ability to travel throughout New Castle County are also required.
