Seeking experienced RN case manager for in-home clinical oversight and patient care. Conduct assessments, develop care plans, ensure compliance, and supervise aides in a homecare setting. Requires active RN license in New Jersey, recent acute or home care experience, and strong clinical skills.

Responsibilities

The Registered Nurse Case Manager conducts comprehensive in-home nursing assessments and develops individualized plans of care for clients. They also supervise Home Health Aides and ensure clinical documentation is accurate and compliant with regulations.
