Job summary

Provide personalized home health nursing care to elderly or terminally ill patients, including assessments, medication guidance, and coordination with healthcare providers, while managing patient care plans and participating in on-call rotations.

Responsibilities

Assess patients in their homes, manage physician-directed plans of care, coordinate with providers and staff, and educate patients about medications. Support daily living needs, document patient status accurately, and participate in the assigned on-call rotation.

Qualifications
