Home Health Registered Nurse Per Diem position involves assessing, planning, and providing nursing care in patients' homes. Responsibilities

Collaborate with an interdisciplinary team to provide high-quality nursing care and comprehensive assessments within patients' homes. Manage the full patient lifecycle from admission and care plan formulation to ongoing monitoring and discharge.

Qualifications

Must be a graduate of an approved professional nursing school and hold a valid Registered Nurse license. Two years of professional nursing practice or community health experience is preferred, with a bachelor's degree desired.
