Job summary

The RN Care Manager designs person-centered care plans, mentors caregivers, and ensures quality and compliance for seniors at home, working independently with a flexible schedule and collaborating with healthcare professionals.

Responsibilities

The RN Care Manager will conduct in-home assessments, develop individualized care plans, and mentor caregivers to ensure high-quality, person-centered care. They will also monitor patient health, maintain compliance with state regulations, and coordinate care with physicians and families.

Qualifications

Candidates must hold an active Registered Nurse (RN) license in New Jersey and possess either a Bachelor's degree in Nursing or related field, or 2+ years of geriatric/home-care experience. A valid driver's license and CPR/First Aid certification are also required.
