Job summary

The role is a hospice Registered Nurse (RN) providing end-of-life care to patients in their homes. The nurse delivers patient-centered, compassionate care, performs assessments, manages symptoms, educates families, and documents care properly. The position involves travel to patient locations, teamwork, and adherence to clinical standards, with a focus on comfort and dignity.

Responsibilities

Provide compassionate hospice care to patients in their homes, including comprehensive assessments, pain and symptom management, and individualized end-of-life care planning. Support and educate patients and families, collaborate with an interdisciplinary team, document care in the EMR, and travel to patient locations within the assigned territory.

Qualifications

Applicants must hold an active Registered Nurse license in the state of employment and have at least two years of RN experience; hospice experience is preferred. A valid driver’s license and the ability to spend approximately 20% of the workday driving to patient locations are required.
