Job summary

Responsibilities

Provide frontline medical care to unhoused populations and individuals with complex behavioral health or chronic conditions in non-traditional settings. Collaborate with a multidisciplinary team to design and implement care plans that support patient stability and improved health outcomes.

Qualifications
