The role involves performing physical assessments, documenting patient needs, consulting with interdisciplinary teams, and developing/revising comprehensive plans of care across the continuum of care. Responsibilities also include coordinating patient activities, leading quality improvement processes, and participating in clinical research.

For Physician Assistants, graduation from an accredited program approved by the NCCPA and one year of primary care management or acute care experience is preferred, along with an NJ Physician Assistant License. Nurse Practitioners require a Bachelor's degree in nursing, a Master's degree in advanced clinical practice or post-Master's certificate, one year of relevant experience preferred, and an Advanced Practice Nurse – NJ License.
