Job summary

Hospice Registered Nurse responsible for delivering end-of-life comfort care, managing symptoms, coordinating interdisciplinary team efforts, and supporting patients and their families with compassion in private, facility, and community settings.

Responsibilities

The Hospice RN will lead patient-centered care by conducting assessments, developing comfort-focused care plans, and managing symptoms. They act as a case manager, coordinating with an interdisciplinary team to provide emotional support and guidance to patients and their families.

Qualifications

Candidates must hold an active RN license and have at least one year of clinical nursing experience. A valid driver's license, reliable transportation, and current CPR certification are also required.
