Job summary

Seeking an experienced RN Case Manager for hospice care in Michigan, responsible for managing patient caseloads, coordinating interdisciplinary care, performing assessments, developing care plans, providing emotional support, and ensuring compliance with regulations.

Responsibilities

The Registered Nurse Case Manager will coordinate interdisciplinary care for a caseload of 12-15 hospice patients. They are responsible for performing comprehensive nursing assessments, developing individualized care plans, and ensuring compliance with Medicare documentation standards.

Qualifications

Candidates must hold an active RN license in the State of Michigan and possess a valid driver's license with reliable transportation. A minimum of one year of hospice or home health experience is preferred along with strong clinical assessment and critical thinking skills.
