Job summary

The Registered Nurse (RN) provides skilled home health nursing, educates patients and families, and documents care while ensuring safety and compliance with regulations. The role involves patient assessment, medication administration, care planning, and supervision of aides and LPNs within state scope of practice.

Responsibilities

Provide skilled nursing care in patients’ homes, assess and monitor their conditions, administer prescribed medications, and develop or adjust individualized plans of care. Educate patients and families, coordinate multidisciplinary care, supervise aides and LPNs, and complete required clinical documentation in compliance with agency and regulatory guidelines.

Qualifications

Requires an active Registered Nurse license in the state of employment and at least one year of RN experience in an acute care setting; home care experience is desired.
