Job summary

An experienced RN will coordinate care, advocate for patients, and evaluate older adults' health and safety needs in a flexible, full-time role, building trusting relationships to improve quality of life.

Responsibilities

The RN Care Manager will serve as a trusted advocate for older adults, coordinating care and navigating complex healthcare decisions. They will conduct in-home assessments, monitor treatment plans, and collaborate with interdisciplinary teams to improve client safety and quality of life.

Qualifications

Assessment Care Coordination Communication Skills Electronic Medical Records Healthcare Navigation Interdisciplinary Collaboration Nursing Patient Advocacy

Candidates must hold an active Ohio Registered Nurse license and possess an Associate's or Bachelor's degree in Nursing. A minimum of 10 years of experience in fields such as Geriatrics, Hospice, or Case Management is preferred.
