The Registered Nurse will perform home health visits, assessments, and admissions while following designated care plans for adult patients. Responsibilities include administering medication, providing wound care, monitoring client conditions, and coordinating care with the multi-disciplinary team.

Qualifications

Case Management Documentation of Care Electronic Medical Records (EMR) Medical/Surgical Nursing Medication Administration Patient Assessment Wound Care

Candidates must hold a current Registered Nurse license in Delaware and possess at least one year of recent clinical medical/surgical experience. Strong computer skills and the ability to travel to assigned cases are required.
