Job summary

The Care Coordinator, in collaboration with the patient/family, social workers, physicians, and interdisciplinary team, ensures patient progression through the continuum of care in an efficient and cost-effective manner. Primary responsibilities include identifying, initiating, and managing patient flow; planning and facilitating care transitions; promoting patient satisfaction and quality outcomes; targeting optimal length of stay; and providing care management planning to ensure quality care, regulatory compliance, and patient/family needs.

Responsibilities

The Care Coordinator manages patient flow and transitions through the continuum of care to ensure efficient, safe, and cost-effective outcomes. They collaborate with interdisciplinary teams to facilitate care planning while maintaining regulatory compliance and patient satisfaction.

Qualifications

Candidates must hold a Bachelor's degree in nursing or a related field and possess a valid RN license. A minimum of 2 years of acute care experience is required for this position.
