Responsibilities

The RN Case Manager performs nursing assessments and administers care to hospice patients in their homes. They coordinate care with an interdisciplinary team and ensure all clinical documentation is submitted in a timely manner.

Qualifications

Candidates must be a graduate of an accredited nursing school and hold a current state Registered Nurse license. At least one year of hospice nursing experience is required, along with reliable transportation and valid auto insurance.
