Job summary

The Care Management Registered Nurse supports patients in a chronic care management program by monitoring patient data, developing care plans, providing clinical support, and collaborating with interdisciplinary teams to improve health outcomes for seniors, primarily through telehealth modalities.

Responsibilities

The Registered Nurse will proactively monitor patient clinical data and vitals to identify care gaps and deliver timely interventions. They will manage individualized care plans and engage with patients via phone and messaging to support chronic disease management and health goals.

Qualifications

Candidates must hold an active Compact Multi-State RN license and possess an Associate Degree in Nursing Science. At least 5 years of recent clinical experience in an ICU or ER setting is preferred, along with technical proficiency in EMR systems.
