Job summary

Seeking a registered nurse to provide home health and hospice care, coordinate patient care with interdisciplinary teams, and support patients and families through end-of-life and palliative services. Requires two years of acute care or nursing home experience, a current WA RN license, and the ability to work independently in various settings.

Responsibilities

Organize and deliver holistic nursing care for hospice and home health patients in various settings. Coordinate with an interdisciplinary team to plan and achieve patient goals while supporting families through end-of-life decisions.

Qualifications

Requires a valid Washington State RN license, BLS certification, and a valid driver's license with a vehicle for travel. A Bachelor's degree in Nursing is required or preferred, along with at least two years of acute care or nursing home experience.
