Job summary

The Licensed Practical Nurse (LPN) provides nursing care and case management to clients, implementing care plans, documenting care, educating patients and families, coordinating home health services, and supporting interdisciplinary team activities under clinical supervision.

Responsibilities

The Licensed Practical Nurse provides direct nursing care and case management to clients and families by implementing prescribed plans of care. They also participate in interdisciplinary team meetings and provide education to patients, caregivers, and direct care staff.

Qualifications

Candidates must hold a current LPN license in good standing and possess a degree in the nursing field. A valid driver's license and the ability to travel daily to client locations are required.
