Job summary

Provides and coordinates nursing case management care for hospice home care patients.

Responsibilities

Provides and coordinates nursing case management for hospice patients while collaborating with a multidisciplinary team. Documents clinical assessments and provides support and education to patients and their families.

Qualifications

Requires a current Texas RN license, basic life support certification, and at least one year of hospice or hospital experience. Candidates must have reliable transportation and the ability to travel to multiple locations.
