Job summary

Family Tree Private Care is seeking a compassionate RN Clinical Care Coordinator to provide in-home assessments, care planning, supervision, and client support. Responsibilities include developing care plans, collaborating with care teams, supervising staff, and maintaining documentation within a home health setting. Responsibilities

The RN Case Manager performs initial assessments in client homes to develop and revise care plans while serving as an account executive for assigned clientele. They also supervise nursing personnel, coordinate services with physicians, and ensure compliance with documentation protocols.

Qualifications
