Job summary

The RN Care Manager assesses and coordinates healthcare needs for seniors, provides nursing care in various settings, collaborates with a team, educates patients and families, and ensures high-quality, individualized care.

Responsibilities

The RN Care Manager assesses participant care needs, provides direct nursing interventions, and collaborates with the interdisciplinary team to develop and implement comprehensive care plans. They also monitor acute and chronic conditions across various settings, including clinics and participant homes, while ensuring regulatory compliance and quality of care.

Qualifications

Assessment Care Planning Communication Infection Control Medication Administration Nursing Patient Care Teamwork

Candidates must hold a valid State RN license and have at least one year of experience providing care as an RN. A Bachelor of Science in Nursing is preferred, along with experience in home care, long-term care, or managed care settings.
