Deliver high-quality post-discharge care for patients with complex medical, behavioral, and social needs. Facilitate safe care transitions, provide patient education, conduct assessments, support remote monitoring, and collaborate with multidisciplinary teams.

Responsibilities

The Registered Nurse will manage post-discharge care transitions to reduce hospital readmissions and provide ongoing clinical support for patients with complex needs.
