Responsibilities include patient assessment, care planning and revision, administering treatments, counseling, education, supervision, and ensuring continuity of care.

Responsibilities

The Registered Nurse plans, organizes, and directs hospice nursing care, utilizing the nursing process to assess and implement patient-centered care plans. They collaborate with the interdisciplinary team to provide symptom management and support to patients and families within the home and community setting.

Qualifications

Assessment Care Planning Communication Skills Compassion Hospice Care Medication Administration Nursing Patient and Family Education Patient Care Teamwork

Candidates must be graduates of an accredited school of nursing with current licensure and CPR certification. The role requires 1-2 years of recent acute care experience and 1-2 years of recent home health or hospice experience.
