The Registered Nurse Case Manager (RNCM) provides leadership and clinical care for patients in a home care setting, including assessment, care planning, medication administration, patient education, documentation, and supervision of care team members. Must have at least one year of RN experience, preferably in acute care or home care, with strong communication, judgment, and teamwork skills.

Responsibilities

The Registered Nurse Case Manager oversees patient care, performs clinical assessments, and develops individualized plans of care. They coordinate multi-disciplinary services, provide skilled nursing, and ensure compliance with Medicare and agency documentation standards.
