Responsibilities

The Home Health LPN provides personalized, one-on-one skilled nursing care to patients in their homes under the direction of an RN. They are responsible for implementing care plans, educating patients and families, and maintaining accurate clinical documentation.

Qualifications

Care Coordination Care Planning Documentation Nursing Patient Care Patient Education

Candidates must hold a current nursing license, a valid driver's license, and current CPR certification. A minimum of two years of experience as an LPN/LVN in a clinical setting is required.
