The RN Care Manager provides clinical oversight and care management for cardiac patients within a multidisciplinary POD team supporting value-based care, risk mitigation, and care transitions. Responsibilities

The RN Care Manager provides clinical leadership for a POD of medical assistants and health coaches to manage cardiac patients under value-based agreements. Responsibilities include clinical triage, high-risk care management, and coordinating transitions of care to reduce hospital readmissions.

Qualifications

Candidates must hold an active, unrestricted RN license and have 3-5 years of clinical nursing experience, preferably in cardiology or population health. Experience with value-based care or risk-based contracts is required.
