Job summary

The role involves providing comprehensive patient assessments, developing individualized care plans based on physician orders, delivering patient-centered care and education, and collaborating with interdisciplinary teams in a home health/hospice setting. The candidate must ensure compliance with regulatory standards, communicate significant patient changes, and promote patient and caregiver autonomy.

Responsibilities

Responsible for determining appropriate care levels and formulating individualized plans of care based on physician orders and CMS regulations. Performs comprehensive patient assessments and provides education to patients and caregivers regarding chronic disease and end-of-life care.

Qualifications
