Job summary

A Licensed Practical Nurse (LPN) supports primary care by providing clinical support, patient care, health screenings, medication administration, and care coordination within a multidisciplinary healthcare team.

Responsibilities

The Licensed Practical Nurse provides clinical support and direct patient care under the supervision of a registered nurse or primary care provider. Responsibilities include performing patient intake, administering medications, assisting with procedures, and maintaining accurate electronic medical records.

Qualifications

Candidates must be graduates of an accredited LPN program and hold a current New York State LPN license. At least one year of clinical nursing experience is preferred, along with current BLS certification and proficiency in EMR systems.
