Job summary

Provide in-home personalized nursing care to patients, conducting assessments, developing care plans, and supporting patients and their families through education and emotional support within a home-care setting.

Responsibilities

The nurse will conduct in-home comprehensive assessments and develop individualized care plans for patients in the Primarily Home program. They will also provide ongoing clinical education and emotional support to patients and their families while collaborating with interdisciplinary teams.

Qualifications

Candidates must hold an associate degree in nursing with at least 3 years of relevant experience and possess a valid registered nurse license in Missouri or Illinois. A valid driver's license and access to reliable transportation are required for home visits.
