Responsibilities

The Center Nurse supports patient health outcomes through clinical assessments, chronic condition management, and the coordination of care plans within a value-based model. They also perform skilled nursing procedures, provide patient education, and collaborate with the interdisciplinary team to reduce avoidable hospitalizations.

Qualifications

Candidates must hold an active, unrestricted Registered Nurse license and possess BLS or CPR certification. Proficiency in English, US work authorization, and experience with Electronic Medical Records are required, with a preference for at least two years of healthcare experience.
