Job summary

Registered Nurse Care Manager provides person-centered, outcome-based care to elders and individuals with disabilities within their homes in Dodge County, coordinating health services, assessments, and care plans as part of an interdisciplinary team.

Responsibilities

The RN Care Manager provides person-centered care management and service coordination for members as part of an interdisciplinary team. Responsibilities include conducting comprehensive assessments, developing member-centered plans, and performing in-home visits to monitor health and service provision.

Qualifications

Candidates must hold a current Registered Nurse license in the State of Wisconsin and possess a valid driver's license. A Bachelor's degree in nursing and at least two years of skilled nursing experience in gerontology, disabilities, or home care are strongly preferred.
