Registered Nurse (RN) Care Manager – Hospice provides skilled nursing care for hospice patients, manages symptoms, coordinates care with interdisciplinary teams, educates families, and supports patient dignity and comfort in private homes and care facilities.

Responsibilities

The RN Care Manager will oversee a caseload of 25-30 hospice patients, providing skilled nursing care and coordinating with an interdisciplinary team to manage symptoms and support families. They are responsible for conducting comprehensive assessments, developing individualized care plans, and ensuring high-quality comfort care in home and facility settings.

Qualifications

Candidates must be graduates of an accredited nursing program with a current Ohio RN license and CPR certification. A valid driver's license, reliable transportation, and the ability to travel throughout the service area are required.
