Job summary

A Care Manager provides care management and service coordination to elderly and disabled members, conducting assessments, developing care plans, coordinating services, and monitoring member outcomes within a team, ensuring compliance with state and federal regulations.

Responsibilities

Assess members’ needs, outcomes, and risks, then develop, coordinate, monitor, and evaluate person-centered care plans with an interdisciplinary team. Coordinate services, benefits, home visits, care conferences, and follow-up care while supporting member rights, safety, independence, and regulatory compliance.

Assessment Care Management Care Plans Communication Computer Skills Medicaid/Insurance Medication Management Patient Education Risk Management Service Coordination

Requires Wisconsin social work certification with at least one year of relevant experience, or a bachelor’s degree or higher with one year of relevant experience in a human services-related field or three years in another field. Candidates must have computer and data-entry skills, a valid driver’s license with acceptable driving history and insurance, and the ability to travel to members’ homes, multitask, and lift up to 25 pounds.
