Responsibilities

The Hospice Home Team Aide performs activities of daily living and personal care for patients in their homes according to an individualized plan. They also provide light housekeeping, meal preparation, and transportation for clients while maintaining clinical records and reporting changes in patient condition.

Qualifications

ADL assistance CNA CPR HHA HIPAA compliance Housekeeping Mobility assistance Patient care Record keeping Transportation

Candidates must hold a valid Florida CNA license or HHA certificate with 75 hours of training and current CPR certification. A minimum of six months of work experience in home health, hospice, or a facility setting is required, along with a valid driver's license and automobile insurance.
