Job summary

Provide one-on-one skilled nursing care to geriatric and adult clients in assisted living, independent living, and senior community settings. Complete assessments, administer treatments, monitor conditions, and coordinate with healthcare teams.

Responsibilities

Provide skilled nursing visits to clients in senior living communities, including comprehensive assessments and individualized care plans. Coordinate care with an interdisciplinary team and maintain accurate clinical documentation using electronic medical records.

Qualifications

Requires a current New Jersey Registered Nurse (RN) license and at least one year of recent clinical nursing experience. Candidates must have reliable transportation to travel throughout Somerset, Middlesex, Monmouth, and Union Counties.
