The Registered Nurse Case Manager in Home Health is responsible for managing patient caseloads, conducting assessments, developing care plans in collaboration with physicians, providing direct patient care, documenting accurately, coordinating discharge plans, communicating with healthcare team members, and participating in quality improvement activities. Requires RN license, BLS certification, at least 3 years of clinical nursing experience, and strong interpersonal, organizational, and clinical skills.

Responsibilities

The Registered Nurse Case Manager conducts initial patient assessments, establishes and updates plans of care, and provides direct nursing care in a home health setting. They also facilitate discharge planning and maintain ongoing communication with physicians and the healthcare team to ensure continuity of care.

Qualifications

Candidates must hold an Associate's Degree in Nursing, a valid Registered Nurse license, and a BLS certification. A minimum of 3 years of clinical nursing experience and a valid driver's license with a reliable vehicle are required.
