Job summary

Seeking an experienced Licensed Practical Nurse (LPN) to provide bedside hospice care, support families, and collaborate with nursing staff in a community-based hospice setting.

Responsibilities

Provide bedside care to hospice patients and offer support to their families and caregivers. Work in close collaboration with the RN Case Manager to ensure quality care delivery.

Qualifications

Automobile Insurance Communication Driver's License Hospice Care Patient Care Practical Nursing Regulatory Compliance Support Families

Must be a graduate of an approved nursing program and hold a valid Ohio Licensed Practical Nurse (LPN) license. Requires 2-4 years of direct patient care experience in hospice or home care and a valid driver's license.
