Job summary

The Hospice RN/Case Manager provides comprehensive care coordination for hospice patients, including assessment, care planning, communication with families, documentation, and interdisciplinary team participation, ensuring compassionate end-of-life care.

Responsibilities

The Registered Nurse will coordinate and provide hospice care by collaborating with an interdisciplinary team to meet the physical, medical, and emotional needs of patients. Responsibilities include ongoing patient assessment, maintaining accurate clinical documentation, and providing education to families regarding the dying process.

Qualifications

Candidates must be a graduate from an accredited School of Nursing and hold an unencumbered Registered Nurse license in the State of Ohio. A minimum of one year of RN experience is required, along with a valid driver's license and reliable transportation.
