A Hospice RN Case Manager provides personalized, patient-centered end-of-life care, managing symptoms, offering emotional support, and coordinating with families and healthcare teams to ensure comfort and dignity for hospice patients.

Responsibilities

The Hospice RN Case Manager assesses patients' health needs, develops and implements care plans, coordinates with healthcare professionals, and provides emotional support to patients and families.

Qualifications

Requirements include a valid RN license, at least two years of nursing experience with a focus on hospice or palliative care, strong assessment and communication skills, and knowledge of hospice regulations.
