Job summary
RN Case Manager provides home-based nursing care, assesses patient needs, develops care plans, educates families, and collaborates with healthcare teams to optimize patient outcomes within the advanced care at home setting.
Responsibilities
Manage and coordinate care for assigned patients receiving Advanced Care at Home, including in-person assessments, care planning, referrals, patient and family education, and ongoing evaluation of treatment and service needs. Communicate changes in patient condition to providers, maintain accurate electronic health records, and contribute to patient safety and quality improvement.
