Job summary
We have immediate opportunities on our Transition Team for Registered Nurses / RNs interested in helping infants, children, and adults receive complex care at home in Chester, Delaware, and Montgomery Counties. The role is FT – 40 hours per week. This team will support the transition of care from the hospital team to ongoing home care services. This role offers a unique opportunity to learn how to care for clients with rare and complex diseases. As part of the Transition Team, you will be responsible for delivering quality care to clients and providing extensive training to the ongoing care team.
Responsibilities
The Transition Team Registered Nurse provides complex care to infants, children, and adults in their homes while supporting the transition from hospital to home care. Additionally, the nurse is responsible for educating and training the ongoing care team to ensure high-quality patient outcomes.
