Job summary

This role involves providing advanced clinical nursing care in patients' homes, managing care plans, coordinating multidisciplinary teams, assessing patient conditions, educating families, and reducing hospital readmissions while maintaining documentation and supporting patient independence.

Responsibilities

The Registered Nurse will conduct comprehensive in-home assessments and manage individualized care plans to support patient recovery and independence. They will serve as the clinical lead, coordinating care across interdisciplinary teams and maintaining accurate documentation in the EMR.
