Job summary

Provide primary and family care in a clinic-based environment, including assessments, diagnosis, treatment, and management of acute and chronic conditions, supporting both in-person and virtual care with collaborative clinical resources.

Responsibilities

Deliver evidence-based primary and family care, including assessment, diagnosis, and treatment of acute and chronic conditions. Perform clinic operations such as diagnostic testing, patient follow-up, and coordination of care in both in-person and virtual settings.

Qualifications
