Providing hospice care to patients in their homes, assisting with daily activities, emotional support, and documentation, while collaborating with a healthcare team.

Responsibilities

Provide compassionate hospice care to patients in their homes, assisting with activities of daily living and supporting comfort, dignity, and quality of life. Observe and report changes in patients’ conditions, reinforce care plans with patients and caregivers, respond to needs, document care in the EMR, and collaborate with the interdisciplinary team.

Qualifications

Activities of Daily Living (ADLs) Compassionate Care Emotional Support EMR Documentation Patient Support

Applicants must have an active Certified Nursing Assistant certificate in the state of employment, a high school diploma or certificate, and at least one year of healthcare experience; hospice experience is preferred. A valid driver’s license and the ability to spend approximately 20% of the workday driving between patient locations are required.
