Job summary

The RN Case Manager manages coordination of home health patient care, ensuring quality, compliance, and effective outcomes by liaising with healthcare teams, reviewing documentation, and overseeing care plans in a Medicare-regulated environment.

Responsibilities

Manage home health patient cases from admission through discharge, coordinating care plans with physicians, clinicians, patients, caregivers, and interdisciplinary teams. Review documentation, monitor outcomes and regulatory compliance, support field staff, and contribute to quality improvement and discharge planning.

Qualifications

Requires an active Texas RN license, at least three years of nursing experience, prior home health case management experience, and experience completing and reviewing OASIS assessments. Candidates should understand Medicare home health regulations, have strong clinical assessment and communication skills, and be proficient with EMR systems; a BSN and WellSky (Kinnser) experience are preferred.
