As an Onsite Care Coordination Nurse, you will support members during care transitions, conduct assessments, support discharge planning, collaborate with healthcare teams, and ensure compliance with safety standards in an acute care setting. Responsibilities

The nurse will conduct face-to-face assessments of newly admitted members at acute care facilities to evaluate health needs and facilitate safe discharge planning. They will collaborate with healthcare providers to identify care gaps and provide education to members and families to improve health outcomes.
